Skilled Nursing Facilities want Medicare A and Managed Care customers! The competition to admit recently hospitalized patients needing a post-acute stay is heating up more than ever and frankly, these patients aren’t excited to be anyone’s customers. No one wants to become ill enough to be admitted to the hospital then to a nursing home to get well and return home (or get trapped and stay).

To capture the post-acute Medicare market and sustain profitability, Skilled Nursing Facilities must clearly understand and put the missing structure into place. This needed structure creates a cycle of success for the critical systems of Admissions, the Right provision of service, Marketing and Sales, and keeps your competition at A.R.M.S. Length. The missing structure also offers hope for recovery to our customers who are often afraid and vulnerable after an illness. The methods used to safely transition patients to our facilities and home again and prevent readmission to the hospital are vital to our credibility as a rehab center of excellence and choice.

This session will also include how to accurately capture the patients’ daily Medicare A Rate, manage Managed Care, and assure the appropriate Length of Stayto assure a safe transition through the implementation of the Team-Based Admission Assessment and Daily Reimbursement/Transition Planning Huddle. To be successful, teams will need to alter many current habits and stifle the cries of, “We don’t have time” when introducing and implementing these critical systems. Therefore, this session will also assist you in effectively introducing change.

Objectives:
At the completion of this session, the attendees will be able to:

  1. Take steps to implement the Rules of 3 to assure a great first impression for newly admitted patients/residents
  2. Implement the Team-Based Admission Assessment and the Daily Reimbursement/Transition Planning Huddle
  3. Take the needed action to involve your staff in a team-based Marketing and Sales effort that will result in Medicare/Managed Care admissions

The targeted audience for this session includes any skilled nursing facility team member involved in working with Medicare A, CEOs, CFOs, consultants to long-term care, Administrators, Directors of Nursing, MDS Coordinators, Therapy team members, Licensed Nurses, Staff Development, Social Workers or Designees, Activity/Recreation, Dietary, Housekeeping, Laundry and Maintenance. This 3 to 4 hour presentation includes lecture, humor, group discussion and individual action planning.
